“Insegnare” l’empatia - “Teaching” empathy

“Insegnare” l’empatia

“Teaching” empathy

I remember the time when Franco had to organise a laboratory on empathy for the students at Isipsé. As it was usual in these cases, his inner state was of a certain agitation, at the same time he was ready to catch any useful suggestion from people around him. He felt the task was challenging, in particular he was concerned that all the students could enjoy the experience. Thinking back, it seems that for him the organisation of the laboratory was not as important as making everyone at ease, both himself and the participants. This was not clear to me at that time, but it comes back now, as something to reflect on. His approach was not only coherent with the concept of empathy (which was the topic of the laboratory) but also addressed the tendency of our time to neglect the importance of feeling at ease. Very much focused on efficiency, performance, productivity as we are, we lose touch with our affectivity,  we become alienated, the “malaise” conquers us. Franco knew the importance of recognising the need of each one to speak and feel at ease; something in him that was not always rightly appreciated. The value of his contribution, however, is unchanged: a seed that has sprouted and keeps doing so. As he taught us, we come back on empathy to keep creating communication between ourselves and the world, to be able to feel at ease, in the therapeutic context and in life.

Empatia che viene dall'altra parte - Empathy coming from the other side

Empatia che viene dall’altra parte

“Prega, sorridi, pensami!

(Henry Scott Holland)

Empathy coming from the other side

Play, smile, think of me, pray for me. Let my name be ever the household word that it always was. Let it be spoken without an effort, without the ghost of a shadow upon it. Life means all that it ever meant. It is the same as it ever was. There is absolute and unbroken continuity…I am but waiting for you, for an interval, somewhere very near, just round the corner. All is well. Nothing is hurt; nothing is lost. One brief moment and all will be as it was before. How we shall laugh at the trouble of parting when we meet again!

EMPATIA: COME CI CURA? (CONTINUAZIONE) - EMPATHY: HOW DOES IT CURE? (CONTINUATION)

EMPATIA: COME CI CURA?
(CONTINUAZIONE)

EMPATHY: HOW DOES IT CURE?
(CONTINUATION)

-        Heinz Kohut believed that the natural tendency of the patient toward self-healing is promoted by the therapist’s empathy and understanding, that helps in removing the obstacles to the natural tendency to self-righting (Pancheri & Paparo). In the relationship with the patient, the therapist must accept some idealization by the patient, who encounters, sometimes for the first time in his life, somebody listening to him with attention and deep acceptance.
-        The therapist must coherently pursue an attitude of mutuality in his counter-transference (following D.Orange I would call  it co-transference)
-        If the couple enters in a state of syntony we can have evidences of Lachmann’s transformational model.

It is difficult to comment on this slide from Franco, so rich and full of references. Actually it would be nice if some reader of this blog could help, to go deeper. I like very much the nice feeling that it conveys of the therapeutic relationship as a place and an opportunity of exchange. “Mutuality” of idealisation as a way of seeing the best in the other. A kindness of regard that can last, even after going through a lot of difficult experiences, and which can make the fatigue of therapy seem light.

Transformation is in therapy a “couple affair”, something that stems from a deep and deeply felt relationship with another person, just like in everyday life. It is interesting to note that all the effort is in “removing the obstacles to the natural tendency to self-righting”. A very modern idea that is finding its way: we are not “wrong” or “sick”. All of us, we just need a little bit of human connection to feel good and work well.

Come cura l'empatia - How does empathy cure

L’empatia: come ci cura?
-        Contenimento
-        Condivisione affettiva
-        Accettazione da parte di una figura idealizzata
-        Rinforzo della coesione del Sé
-        Esperienza emozionale correttiva

Empathy: how does it cure?
-        Containment
-        Emotional sharing
-        Acceptance by an idealized figure
-        Strengthening self-cohesion
-        Corrective emotional experience
-        Creation of new organizing principles

I am not going to comment today, I’m exhausted by too much work and it comes to my mind that Franco would have disapproved such work exaggeration. He is always very near to us all, his words on empathy once again so simple and rich.

Apprezzando profondamente – Deeply appreciating

In ogni caso molto coinvolgente.

The Cambodian version of L.U.D.A. – by Franco, uncertain dating (anybody knows?)

This is what Franco has called the Cambodian concept of empathy. Working with Cambodian refugees, women refugees who had a role as nurses, Franco received from them a new and interesting version of his formula about empathy. Listening and understanding here go hand in hand with deep appreciation. We could say that deep appreciation includes deep acceptance and that this formula could be seen as more complete, even if less explicit than the original one. In any case a strong message. It suggests a kind of relationship which creates a very deep connection and which is down the line extremely therapeutic.

Empatia: Heinz Kohut - Empathy: Heinz Kohut

EMPATIA: Heinz Kohut

EMPATHY: Heinz Kohut

-          Kohut’s Contributions
o   Highlighted other aspects of empathy, such as psychological nourishment

-          Meaning of psychological nourishment:
o   “A bond between human beings more powerful than love to contrast destructivity and dehumanization”

-          Kohut considered psychological nourishment one of the most important ideals to pursue

-          Kohut felt that empathic understanding of difference may help to avoid social conflicts and wars

-          IN THIS SENSE, Kohut felt empathy was essential for human survival

Kohut is the first psychoanalyst to place empathy at the centre in the relationship. For me he is also the first psychoanalyst who brought to psychoanalysis the colour of a deep sense of humanity. It was a missing colour in the picture and reading his books one can feel the stress and labour he undertook to try to harmonise this colour with the other existing ones. Franco followed him on this route and he carried on in Italy Kohut’s approach with patience and perseverance. Reading Kohut and particularly what he says about empathy, is a comforting empathic experience: in his words there is such a profound understanding of the human being and of what is being sought in the therapeutic relationship. Franco fully perceived the importance of Kohut’s message and became a messenger making available an essential psychological nourishment for us all.

La dolcezza della presenza - The sweetness of presence

The sweetness of presence

Empathy is a quality of human experience which makes life bearable, desirable and finally loved. To be present for the other is the greatest hope that we can donate regarding the importance that the other has for us. But often we are not aware of this, we undervalue the preciousness of just our presence for the other and we miss many occasions for demonstrating our love. We lack empathy towards ourselves and our presence becomes poorer.
When a person leaves us, like Franco, his empathic presence remains, in my opinion. It might seem a strange phenomenon, nevertheless, within ourselves, the empathic presence of the other is as eternal, it is something which does not fade away.
Christmas is getting near, a celebration which Franco tried to escape, a moment for being together. Sometimes it isn’t possible to be present, but the sweetness of memories is always there.

EMPATIA: Carl Rogers - EMPATHY: Carl Rogers

EMPATIA: Carl Rogers

EMPATHY: Carl Rogers
Carl Rogers’ Definition:

The extent to which the therapist:
1) is sensitive to the current feelings and thoughts (both those in and those out of awareness) of the client
2) has the ability to communicate his understanding of his client’s feelings and thinking, using language attuned to that of the client
Empirical research supports Rogers’ thesis, supporting the idea that empathy per se is therapeutic.

Reading this definition Rogers’ genius shines through. It is wonderful to continue this journey with Franco on the path of empathy. With his simplicity, he left us such a precious inheritance: each fragment of his discourse on empathy is like a mantra to meditate on, for increasing our awareness of the roads we need to build for a more human life, based on empathic relationships.

Empatia forever - In memoriam

Empatia
Origine:

Empathy
Origin:
- Coined by Tichener in 1909
- Derived from the Greek word empatheia, and is similar  to the word sympathy.
The term originally involved perceiving and understanding the non-human, humanizing it through one’s own feelings.”

To feel somebody or something, a situation, certain relationships…to feel being able to take on the other one’s perspective. Developing the quality of what is humane in ourselves, to be able to better connect to others, it’s a job for life and beyond.

Centralità dell'empatia - Centrality of empathy

Centrality of empathy

"For the traumatized person the first faculty to recover is the basic capacity of trust. This may be possible only through the empathic attitude of the psychotherapist and his authentic reliance on the patient’s resources.
Remember always that “the patient is the first expert of himself” (Paparo) and the first actor of his recovery."

It is somehow moving to read these words from Franco. Simple words, so profound.  Trust is such a fundamental human dimension and it is this very basic aspect of life that has been broken down when trauma occurs. May be, if we really managed to understand this and its importance for all human beings - not just the ones directly affected - we would be able to start “mending”  the injury that the humanity as a whole is suffering today. In this path, each one is an expert – we can all be first actors.

Le strategie psicoterapeutiche con le persone traumatizzate: l’importanza fondamentale dell’empatia - The psychotherapeutic strategies in traumatized people: the fundamental importance of empathy

The psychotherapeutic strategies in traumatized people: the fundamental importance of empathy

Since the central experiences of psychologically traumatized patients are ”disempowerment and disconnection” (Herman, 1992), the healing must be based on giving some power to the patient and favour  some connection first of all with the person of the therapist, who has to be able to listen empathically, since healing cannot occur in isolation and can take place only in the context of  relationships.

The therapist and the connection with the patient, it’s a topic evergreen, which doesn’t only concern traumatised people. How to define this connection and how to nurture it? Come to mind some seminars that Franco organised for the students of the School of Psychotherapy (ISIPSè) and the questions that were puzzling him about how to work with them on empathy. I have the feeling that while Kohut’s and Franco’s generation discovered the fundamental role of empathy, it will be the younger generations who will actually live by it and possibly this will bring a strong reduction of traumatised people in the world. In fact, we can imagine that a diffuse empathic “competence” will reduce the kind of conflicts that bring so many traumatic experiences.

Nuovo modello medico Bio- psico- socio- spirituale - New medical model: Bio--psycho-social-spiritual

Nuovo modello medico Bio- psico- socio- spirituale

^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^^

To overcome the gap between common medical approach and the patients needs, particularly for traumatised patients, Franco proposes a new medical model:

New medical model: Bio--psycho-social-spiritual

It incorporates:

- Research

Framework for teaching

- An empathic approach to let the patient experience the real world of suffering pursuing ,with the doctor’s help, the aim of improving his life

- Medical caretaker not familiar with this new model, in order to help traumatized patients’ self-healing, should be re-trained (as Richard Mollica has done rather successfully with Cambodian general practitioners and nurses).

I think it is so important that Franco brings together empathy, research, a framework for teaching and the training or re-forming of the traditionally trained doctor. From my point of view, the great value of this proposal is in the sentence "let the patient experience the real world of suffering pursuing ,with the doctor’s help, the aim of improving his life". An operation which is far from easy to perform, given the complexity of the necessary conditions for its implementation: personal therapists conditions and healing environment's and wider context's conditions. I believe the experience of sharing on this issue by the therapists could be an excellent starting point for their re-training: how one feels in the presence of someone else's suffering, what empowers one in "assisting" it. Each and everyone has his/her relationship with suffering. For a therapist the story about this relationship gets intertwined with the way he/she sees the relationship with someone else's suffering. The "simple" act of being near to the other who is suffering is mediated by these aspects which are not easily brought into awareness.

Biodiversità ed empatia - Biodiversity and empathy

Biodiversity and empathy

There is a lot of talk, nowadays, about the loss of animal and vegetable species linked to intensive, standardised and “privatised” agriculture which serves the interests of multinational agri-food companies (see note). I wonder what the relationship is between this loss of “diversity” and the ever increasing trend to the standardisation of human activities. Starting with birth, then school environment and continuing with working places, perhaps only when pensioning  human beings seem to come to have some right to living their “biodiversity”. And for younger generations even this possibility is at risk.
Franco, for instance, decided to pension rather early and in this way he could give room to what was special and different in him, to his creativity. This in time brought fruits such as the creation of a strong psychology of the Self Italian group, contributing to the dissemination of some fundamental concepts for the human being.
Difference is an important value and resource and it is typically human to note and appreciate it. As Franco says relating back to Kohut, empathic understanding of difference may help to avoid social conflicts and wars. At a moment when the  great needs of an ever increasing number of human beings are challenging us, recognizing the value of diversity could be vital. Through empathy we can safeguard human “biodiversity” and this could probably help us protect animal and vegetable biodiversity too.

Just two examples: since the beginning of the 20th century, 75% of vegetable varieties has disappeared. In Italy at the end of the 19th century there were 8.000 varieties of fruit, only 2.000 are left.

Empatia - Empathy

Empatia – Empathy

I remember when Franco used to tell me about his encounter with some Vietnamese women participating to the Harward programme on social trauma. To this women Franco had communicated his guide for approaching and helping  suffering people, which was summarised in the acronym LUDA: Listening, Understanding, Deeply Appreciating (or Accepting, sometimes we couldn’t remember but it’s good either way!).

These women were enthusiastic about his work, about what he managed to communicate to them. And he was enthusiastic about their capacity to get hold of his teachings but also of how they felt supported by his teaching and his presence.

Being there for the other, it was something that Franco was able to do naturally.

Franco e l'empatia - Franco and empathy

I think that a clear demonstration of the importance of the concept of empathy for Franco, has been his cleverness in understanding how significant Kohut’s work was and his dedication to get him translated in Italian and disseminated.
He knew how important this was in the “psy” community in Italy, he knew by instinct. It was a necessity, a need felt by many, to find a psychoanalytic author able to speak of empathy and of it’s crucial role in the psychotherapeutic relationship. Although most of us didn’t know beforehand what exactly we needed, I think that when reading Kohut for the first time many of us thought: this is what I was looking for, this is what was missing.
Franco hasn’t just understood the importance of Kohut’s work, above all he understood how much we needed it, how much we needed those words, those discourses and thus he gave a demonstration of deep empathy. And with utmost humbleness he engaged for bringing Kohut’s work in Italy: translating, explaining, make you a part of it.
