Co-Costruzione di una Relazione Empatica

Modalità tipica di intendere il concetto:

-        -   Il terapeuta contribuisce alla comprensione empatica
-        -    Il paziente contribuisce con la propria disponibilità a essere compreso empaticamente

La co-creazione dell’empatia plasma l’atmosfera affettiva della relazione terapeutica e trasforma gradualmente l’esperienza per entrambi i partner


E’ difficile capire questo concetto, che cosa voglia dire in pratica se non lo sviluppo di un rapporto paritetico, sempre più paritetico man mano che cresce la consapevolezza di quel che succede nella relazione. Mi pare che sia particolarmente cruciale il concetto di come l’empatia sia trasformativa dell’esperienza “per entrambi i partner”.

-          - Therapist contributes empathic understanding
-           - Patient contributes availability to be empathically understood

Co-creation of empathy shapes the affective ambience of the treatment and gradually transforms the experience for both partners


It is difficult to understand this concept, its more prominent practical implications probably relating to the development of an equal relationship, which becomes more and more equal as awareness increases about what is happening between in the relationship itself. Particularly crucial appears the concept of empathy as being transformative of the experience “for both partners”.

Ottobre per te – October for you




Ottobre per te 

Ottobre era il mese in cui eri rimasto orfano di padre. La mamma vedova con due bambini, di cui tu eri il più piccolo. La perdita, l’abbandono e l’ansia che rimane. Un evento che ha cambiato il corso della vita e che forse ha trovato un sollievo solo quando hai incontrato Kohut. Una persona gentile che ha dato una risposta ai bisogni di tante persone e che tu hai voluto far conoscere a tutti. Nell’andare oltreoceano ad incontrarlo hai pianto sull’aereo come una fontana, forse il padre che non avevi mai avuto. Poi conservavi con cura un piccolo prezioso bigliettino con cui ti aveva dato appuntamento. Frammenti di una storia d’affetto che fa grande la scienza.


October for you


October was the month when your father had died leaving you an orphan. The mother was left a widow with two children, you were the younger one. Living with the loss, abandonment and lingering anxiety. An event which changed the course of life and perhaps only found relief when meeting Kohut. A gentle person who has given an answer to the needs of many, a person you wanted everyone to discover. Going to the other side of the ocean to meet him, you cried “like a fountain”, perhaps the father you had always missed. Afterwards, you carefully stored a small precious note he had sent you for an appointment. Fragments of affectionate devotion which make science great.

Imparare dai pazienti

Franco era molto umile e pronto ad apprendere dai pazienti. Aveva sempre come un’attitudine di interesse, curiosità, era come se quel che il paziente poteva portare fosse sempre tutto nuovo, qualcosa da scoprire. Penso che riuscisse ad ascoltare per il piacere della novità, l’idea che ci fosse qualcosa da aspettarsi nell’incontro con l’altra persona. E quindi era molto aperto a imparare, a capire o meglio a non capire, a poter stare con l’altro anche senza capire. Per imparare che cosa succede, come una persona si muove e poi risolve, ma senza ansia di aiutare e senza senso di colpa per la propria possibile inadeguatezza.


Learning from patients


Franco was very humble and quick to learn from patients. He had always an attitude of interest, curiosity; it was as if what the patient could bring was always completely new, something to discover. I think he was so good at listening for the pleasure of novelty, the idea that there was something to expect from the encounter with the other person. So he was very open to learn, to understand or better still to not understand, to being with the other also doing without understanding. To learn what happens, how a person moves and solves problems, but without feeling any pressure to help or any guilt feelings for his own potential limits.

Ancora sulla "rottura"

Ne parlavo a gennaio, del concetto della "disruption" caro a Franco. E' un concetto estremamente importante e utile sia nella pratica clinica che nella vita quotidiana. I nostri rapporti si reggono spesso in un precario equilibrio tra conflitti e armonie. Quando questo equilibrio si perde si precipita nella "disruption".
Ecco, il concetto di cui parliamo comporta che questo abisso non sia più un abisso. Che laddove tutto sembra perduto ci sia invece ancora la possibilità di recuperare. Comporta lo sviluppo di un nuovo modo di pensare, di una nuova cultura. Una mentalità in cui possiamo provare a concepire la rappacificazione anche quando normalmente andremmo verso una separazione, un allontanamento senza rimedio.
 Credo che questo presupponga il nascere di un nuovo tipo di rapporti a cui forse non siamo così preparati. Soprattutto il penetrare di un approccio di pensiero positivo che riesce a vedere il lato produttivo anche delle esperienze più problematiche.
Allontanarsi e poi riavvicinarsi, avendo capito qualcosa di importante proprio attraverso la rottura. E' un gesto di fiducia, un dare e darci una possibilità. Una bella idea da sperimentare e poi raccontare.


Disruption once again

I was writing in January, about the concept of "disruption" which was very dear to Franco. It is an interesting and useful concept, both for clinical practice and for every day life. 
Our relationships often keep up in an uncertain balance between conflicts and harmonies. When balance is lost, we precipitate in "disruption". Now, the concept we are talking about implies that this abyss is not an abyss anymore. It implies that when everything seems lost, there is still the possibility to recover. It implies the development of a new way of thinking, of a new culture. It gives us the possibility to think in terms of reconciliation also when we would normally go towards a separation, a final moving away. 
I think that this brings with it the birth of a new kind of relationships, to which we are probably as yet unprepared. Above all it carries a positive thinking approach, which can manage to see the productive side of some of the most problematic experiences.
Moving away and coming back, having understood something important thanks to the disruption. It's an act of trust, a giving to ourselves and to our life companions a possibility. A good idea to experiment with and later tell about.

-        Heinz Kohut era convinto che la tendenza naturale del paziente verso l’autoguarigione fosse sostenuta dall’empatia e dalla comprensione del terapeuta, che aiuta a rimuovere gli ostacoli che si frappongono alla naturale tendenza al self-righting (Pancheri & Paparo). Nella relazione con il paziente, il terapeuta deve accettare una certa dose di idealizzazione da parte del paziente, il quale gode, magari per la prima volta nella sua vita, della presenza di qualcuno che lo ascolta con attenzione e profonda accettazione.
-        Il terapeuta deve coerentemente perseguire un atteggiamento di mutualità nel suo contro-transfert (ispirandomi a D.Orange lo chiamerei co-transfert).
-        Se la coppia entra in sintonia, troviamo evidenza del modello trasformazionale di Lachmann.

E’ difficile commentare questa slide di Franco, così ricca e piena di riferimenti bibliografici. Sarebbe bello se qualcuno dei lettori del blog potesse dare una mano, per approfondire. Mi piace molto la bella sensazione che esprime a riguardo della relazione terapeutica come luogo e opportunità di scambio. L’idealizzazione “mutua” per vedere sempre il meglio nell’altro. Una gentilezza dello sguardo, del modo in cui si guarda all’altro, che può resistere anche al fatto di dover attraversare tante difficili esperienze, e che può far sembrare leggera la fatica della terapia.
La trasformazione in terapia è un “affare di coppia”, qualcosa che scaturisce da una relazione profonda e profondamente sentita con un’altra persona, proprio come nella vita di tutti i giorni. E’ interessante notare che tutto lo sforzo si concentra nel “rimuovere gli ostacoli che si frappongono alla naturale tendenza al self-righting”. Un’idea molto moderna che si sta facendo strada: non siamo “sbagliati” o “malati”. Tutti noi abbiamo semplicemente bisogno di un po’ di connessione umana per sentirci bene e lavorare con soddisfazione.

-        Heinz Kohut believed that the natural tendency of the patient toward self-healing is promoted by the therapist’s empathy and understanding, that helps in removing the obstacles to the natural tendency to self-righting (Pancheri & Paparo). In the relationship with the patient, the therapist must accept some idealization by the patient, who encounters, sometimes for the first time in his life, somebody listening to him with attention and deep acceptance.
-        The therapist must coherently pursue an attitude of mutuality in his counter-transference (following D.Orange I would call  it co-transference)
-        If the couple enters in a state of syntony we can have evidences of Lachmann’s transformational model.

It is difficult to comment on this slide from Franco, so rich and full of references. Actually it would be nice if some reader of this blog could help, to go deeper. I like very much the nice feeling that it conveys of the therapeutic relationship as a place and an opportunity of exchange. “Mutuality” of idealisation as a way of seeing the best in the other. A kindness of regard that can last, even after going through a lot of difficult experiences, and which can make the fatigue of therapy seem light.

Transformation is in therapy a “couple affair”, something that stems from a deep and deeply felt relationship with another person, just like in everyday life. It is interesting to note that all the effort is in “removing the obstacles to the natural tendency to self-righting”. A very modern idea that is finding its way: we are not “wrong” or “sick”. All of us, we just need a little bit of human connection to feel good and work well.

Come cura l'empatia - How does empathy cure

                                 


L’empatia: come ci cura?

-        Condivisione affettiva
-        Accettazione da parte di una figura idealizzata
-        Rinforzo della coesione del Sé
-        Esperienza emozionale correttiva
-        Creazione di nuovi principi organizzativi

Oggi non farò commenti, sono sfinita dal troppo lavoro e mi viene in mente che Franco non avrebbe approvato una tale esagerazione lavorativa. Lui è sempre vicino a tutti noi, le sue parole sull’empatia ancora una volta sono talmente semplici e ricche.

I am not going to comment today, I’m exhausted by too much work and it comes to my mind that Franco would have disapproved such work exaggeration. He is always very near to us all, his words on empathy once again so simple and rich.

Una diversa qualità dell’ascolto

Ascoltare attentamente

1.      Prestare attenzione alle proprie emozioni, monitorarle
2.      Comprendere
3.      Sintonizzarsi sui bisogni del paziente
4.      Accettare profondamente il paziente
5.      Impegnarsi affettivamente in maniera ottimale

Franco preparava con una certa ansia le sue presentazioni, a volte negli ultimi tempi facendosi aiutare da qualcuno più esperto con il powerpoint. La presentazione di cui fanno parte i testi che ho citato nella gran parte degli ultimi post la preparò alla fine del settembre 2007, lavorando “come un matto”.
“Una diversa qualità dell’ascolto” era davvero una cosa speciale di Franco, che penso molte persone abbiano apprezzato di lui. Sebbene fosse a volte un gran chiacchierone, quando ascoltava lo faceva con grande rispetto ed anzi era singolarmente silenzioso, nel senso che non era da lui fare commenti – solo qualche ipotesi ogni tanto, con molta cautela.
La sua proposta non è facile da mettere in pratica, ma coinvolge. A differenza di molti altri, Franco aveva il coraggio di dichiarare in parole semplici cosa occorre fare con un paziente – senza però mai dire a nessuno cosa doveva fare. Spero che ci siano allievi che seguano il suo esempio, perché davvero c’è tanto bisogno di chiarezza e semplicità che aiutino gli psicoterapeuti ad orientarsi e trovare la giusta strada per aiutare chi ha bisogno.

Franco prepared his presentations with a certain anxiety. Lately sometimes getting somebody to help him for working with powerpoint. He prepared the presentation from which are taken the citations of most of the last posts at the end of September 2007, working “as a fool”.
“A different quality of listening” was really something special in Franco. I think many people appreciated this quality of his.  Although sometimes he could be quite talkative, when he listened to you he did it with such respect and quietness – he would not make comments of any kind, just an hypothesis here and there, with much cautiousness.
His suggestion isn’t easy to practice, but calls to get involved. Differently from many others, Franco had the courage to say in simple words what needs to be done with patients – without however ever telling anyone what they should do. I hope his alumni will follow his example, because there is really need of clarity and simplicity  to help psychotherapists find the way to help those in need.
