-          No allo psicoterapeuta silenzioso

-          No allo psicoterapeuta come schermo neutro che non reagisce alle rivelazioni del paziente. No alla “neutralità morale bensì neutralità tecnica”(Herman)

-          No allo psicoterapeuta che impiega una modalità poliziesca per cercare di creare una storia  plausibile a partire dal materiale portato dal paziente

-          Considerazione della cosiddetta resistenza come di un modo del paziente di difendere la sua personalità e identità

-          L’intervento psicoterapeutico potrebbe consistere di una singola seduta, una serie corta o lunga di sedute, a seconda del bisogno del paziente di questo tipo di aiuto e della qualità dell’incontro e non sempre di un lungo periodo di sedute a frequenza fissa.”


Franco proponeva un approccio flessibile alla terapia in base alle necessità del paziente, alle situazioni, ai contesti e anche alle preferenze del terapeuta rispetto al lavorare con setting diversi. Era molto sensibile al comportamento verbale e non verbale dei pazienti e penso che questa fosse una delle ragioni che lo rendevano così attento alle differenze nei bisogni di pazienti appartenenti a contesti diversi, con varia cultura, con motivazioni diverse nel cercare l’aiuto psicoterapeutico. Credo che avesse sviluppato questa sensibilità quando lavorava al manicomio, dove molte cose avvenivano sotto la superficie di ciò che poteva essere detto a parole. Il trauma era qualcosa che lui conosceva così bene, penso fosse stato lui stesso traumatizzato dal vivere e lavorare con i pazienti psichiatrici. Era traumatizzato dai trattamenti che i pazienti ricevevano, dalle condizioni in cui dovevano vivere, dal fatto che si trattava spesso di povera gente che nessuno si preoccupava di proteggere. Franco fece del suo meglio per migliorare le condizioni del manicomio, anche inserendo tante persone giovani per farsi aiutare. Era il suo modo di rispondere al trauma quotidiano della vita in manicomio. Le sue idee per una psicoterapia del trauma non potevano che rispettare la totale libertà di cui i pazienti hanno bisogno per poter trovare la propria via d’uscita dall’esperienza traumatica e per andare verso una vita nuovamente apprezzabile.

-          No to the blank screen with psychotherapist not affected by patient’s revelations. No “moral neutrality but tecnical neutrality”(Herman)

-          No to the psychotherapist employing a detective method  to fashion from the patient’s material a plausible narrative

-          Consideration of so called resistances as a  way of the patient to defend  his personality and identity

-          Psychotherapeutic intervention might consist of a single session, a short or a long series of sessions, according to the patient’s need of this type of help and the quality of the encounter and not always of a long period of sessions at a  fixed frequency”

Franco proposed a flexible approach to therapy in relationship with the patient’s needs, situations, contexts and also the therapists preferences for working in different settings. He was very perceptive of the patients verbal and non-verbal behaviour and I tend to think that this was one of the reasons that made him so sensitive about the differences of the needs of patients belonging to different contexts, backgrounds, reasons for seeking psychotherapeutic help. I think he learnt about this when working in the psychiatric clinic, where so much went on under the surface of what could be said in words. Trauma was something he knew so well, I think he was himself traumatised by living and working with mental patients. He was traumatised by the treatment these patients received, the conditions they had to live in, the fact that it was the poor people who found themselves in the hospital and there was no way to protect them. Franco tried his best to improve the conditions of the clinic, also bringing a lot of young people in to help him. It was his way to answer to the daily trauma of the life in a psychiatric ward. His ideas for a psychotherapy of trauma could not but respect the total freedom that patients need to be able to find their way out from the traumatic experience to an enjoyable life.

"La psicoterapia intersoggettiva secondo questo nuovo modello

La psicoterapia dinamica intersoggettiva per me oggi non consiste in una situazione, in cui è possibile, insieme al paziente, provare a capire che cosa è andato male nella sua infanzia e vita precedente – sebbene alcuni aspetti di una nuova visione da parte del paziente possano essere centrali per l’efficacia del trattamento – ma piuttosto in un tentativo di ascoltare il paziente e raggiungerlo, offrendogli una relazione attraverso la quale può diventare consapevole delle sue difficoltà e risorse.
In altre parole, lo psicoterapeuta deve mettere in moto un processo in cui il paziente riesce a usare le proprie risorse per nuove scoperte e collegamenti."

Vale la pena sottolineare il focus di Franco sulle risorse del paziente – un aspetto così spesso trascurato in campo psicoterapeutico, talmente i problemi finiscono per ingombrare tutto il campo. Penso ci sia ancora molto da scoprire in quest’area di grande interesse, sia nel senso di quanto in effetti sia il paziente, grazie alle proprie risorse, a guidare la terapia; sia nel senso di quanto è utile nella terapia parlare delle difficoltà vs. vissuti positivi e quale equilibrio tra i due aspetti possa essere ottimale.

"Intersubjective psychotherapy according to  this new model

Dynamic intersubjective psychotherapy for me today does not consist in a situation, in which is possible, together with the patient, to try to understand what went wrong in his infancy and previous life-although some aspects of a new vision by the patient may be central to the efficacy of the treatment- but as an attempt to listen to the patient and  reach him, offering him a new relationship through which he becomes more aware of his difficulties and resources.
In other words, the psychotherapist must set in train a process in which the patient becomes able to use his own resources for new  discoveries and connections."


It is worth stressing out Franco’s focus on the patient resources – something often forgotten in the psychotherapeutic  field – usually problems take up all the scene. I think there is still a lot to discover in this very interesting area, both in terms of how much is the patient him/herself guiding  the therapy, thanks to own resources; and in terms of the usefulness, in therapy, of speaking about problems vs. positive feelings and the relative balance between the two aspects.

Franco e il treno - Franco and the train




Franco e il treno
A Franco piaceva viaggiare in treno: leggere, studiare e infine arrivare. Più in generale gli piaceva essere trasportato. Più o meno sui sessant’anni aveva deciso di non guidare più. La cosa gli metteva troppa ansia e troppe grane con la macchina, le multe…E così si muoveva con i mezzi pubblici o appunto gli piaceva essere “scarrozzato” in giro dagli amici e dai colleghi. Il treno era amato per la possibilità di studiare in santa pace e quasi non accorgersi della fatica del viaggio.
Anche a me il treno piace per lo stesso motivo, specialmente quando è confortevole e il vagone silenzioso. Eppure non credo di apprezzarlo come faceva lui che se lo godeva proprio, forse perché apparteneva a una generazione che riesce ancora a fare le cose con calma. Noi oggi tutti di corsa ma in fondo anche il treno superveloce appartiene a un altro mondo, è ancora attaccato alla terra.
In comune abbiamo tutti l’ansia del viaggio, della partenza che porta alcuni a giocare d’anticipo e altri a rincorrere, sempre in ritardo.
Franco viaggiava leggero, un bagaglio essenziale, forse per questo il suo viaggio così a lungo ha potuto durare.

Franco and the train
Franco was fond of traveling by train: reading, studying and arriving at last. More in general he liked to be transported around. When about sixty he decided not to drive anymore. It made him anxious to drive in the city, too many problems with the car and fines…So he preferred public transport or being run around by friends or colleagues. He loved trains because they allowed him quiet studying and traveling without too much fatigue.
I like trains for the same reason, especially comfortable and silent ones. Still, I don’t think I can appreciate them as he did, enjoying them; perhaps due to the fact that he belonged to a generation which still manages to do things with calm. We are all on the run today but after all, trains, even high speed ones, are part of another world, a world still rooted to the earth.
What we all have in common, though, is some traveling anxiety, to which some of us react playing in advance and others always running after, always delayed…

Franco used to travel with a light luggage, an essential one, this perhaps explains why his journey was long lasting.

Nuovo modello medico Bio- psico- socio- spirituale - New medical model: Bio--psycho-social-spiritual


Per superare il divario fra comune approccio medico ed esigenze dei pazienti, in particolare per la cura dei pazienti traumatizzati, Franco propone un nuovo modello medico:

Nuovo modello medico Bio- psico- socio- spirituale

che comprende:

- la ricerca

- una contesto di insegnamento

- un approccio empatico che permette al paziente di esperire la sofferenza mentre, con l'aiuto del medico, cerca di migliorare la sua vita

- il medico che non ha familiarità con questo nuovo modello, per aiutare l'autoguarigione dei pazienti traumatizzati, dovrebbe essere formato nuovamente (come è stato fatto da Richard Mollica e con successo, con i medici e le infermiere cambogiane).

Credo sia da sottolineare l'importanza dell'aver unito in un'unica ricetta l'empatia, la ricerca, una cornice di insegnamento e la formazione o ri-forma del medico tradizionalmente formato. Dal mio punto di vista, il grande valore di questa proposta è nella frase "permettere al paziente di esperire la sofferenza mentre, con l'aiuto del medico, cerca di migliorare la sua vita". Un'operazione questa, assai difficile da condurre, per la complessità delle condizioni  necessarie alla sua attuazione: personali del terapeuta e più ampiamente sia dello healing environment sia del contesto più ampio. Credo che la condivisione dei terapeuti su questo punto potrebbe essere un punto di partenza per la loro ri-formazione: come ci si sente di fronte alla sofferenza dell'altro e cosa fa sì che la si possa "assistere". Ognuno di noi ha una storia di relazione con la sofferenza, e per un terapeuta questa storia va a intrecciarsi con il modo in cui concepisce il rapporto con la sofferenza dell'altro. L'operazione "semplice" di stare vicino all'altro che soffre è mediata da questi aspetti di cui non è facile avere consapevolezza.

To overcome the gap between common medical approach and the patients needs, particularly for traumatised patients, Franco proposes a new medical model:

It incorporates:

- Research


Framework for teaching

- An empathic approach to let the patient experience the real world of suffering pursuing ,with the doctor’s help, the aim of improving his life

- Medical caretaker not familiar with this new model, in order to help traumatized patients’ self-healing, should be re-trained (as Richard Mollica has done rather successfully with Cambodian general practitioners and nurses).

I think it is so important that Franco brings together empathy, research, a framework for teaching and the training or re-forming of the traditionally trained doctor. From my point of view, the great value of this proposal is in the sentence "let the patient experience the real world of suffering pursuing ,with the doctor’s help, the aim of improving his life". An operation which is far from easy to perform, given the complexity of the necessary conditions for its implementation: personal therapists conditions and healing environment's and wider context's conditions. I believe the experience of sharing on this issue by the therapists could be an excellent starting point for their re-training: how one feels in the presence of someone else's suffering, what empowers one in "assisting" it. Each and everyone has his/her relationship with suffering. For a therapist the story about this relationship gets intertwined with the way he/she sees the relationship with someone else's suffering. The "simple" act of being near to the other who is suffering is mediated by these aspects which are not easily brought into awareness.

Franco e il mare - Franco and the sea




Franco e il mare

Siamo in Agosto, come non pensare a Franco e alle sue nuotate in mare. Con molta cautela si immergeva in acqua per le sue rinvigorenti nuotate e alla fine dell’estate era molto felice di esibire l’abbronzatura, ottenuta senza mai esporsi al sole nelle ore più calde.
L’estate, il mare e un grande sorriso. L’amata Castiglioncello.
Viene in mente anche lo “Healing Environment”. In questo caso un luogo bello in cui tornare per riposarsi e godersi il contatto con la natura, rimettere in sesto il fisico e anche lo spirito.
Chissà, se tutte le persone anziane potessero avere questa possibilità, durante l’estate un luogo ove ristorarsi, cambiare aria e sentirsi felici. Nel mondo della Kirghisia sognata da Silvano Agosti le seconde case potrebbero essere vissute molti mesi all’anno dando la possibilità a molte più persone di godere del sole, del mare, di un respiro di aria bella.

It’s August and it comes natural to think about Franco and his love for being at the seaside. Very cautiously he would get in the water for his invigorating swim and at the end of the summer he was happy to show his perfect suntan, obtained without ever exposing himself in the hottest hours of the day.

Comes to mind also the “Healing Environment”. In this case, a beautiful place to which to go back for resting and enjoying the connection with nature, restoring both body and spirit.

I wonder…if all the aged people would have this possibility, during the summer to have a place to refresh, breath and feel happy. In the world of Kirghisia imagined by Silvano Agosti, summer homes could become alive for many more months a year, providing the opportunity to many more people of experiencing the pleasure of being at the seaside and getting a breath of full, fresh and beautiful air.

Caro Franco, questa canzone è per te. E' uno dei canti di consapevolezza di Thay, il maestro Thich Nhat Hanh che non abbiamo fatto in tempo a visitare insieme. E' anche una canzone per ciascuno di noi, è un mantra profondo che può nutrire il nostro cuore, perché appunto come insegna Thay e tutto il buddismo, non c'è nascita e non c'è morte e quando sappiamo riconoscere in noi la realtà dell'interessere ci avviciniamo a comprenderlo.

Dear Franco, this song is for you. It is one of Thay's mindfulness songs, master Thich Nhat Hanh whom we did not manage to visit together. It is a song for each one of us, it is a profound mantra that can nourish our heart. As Thay and Buddism teach us, there is no birth and no death and when we develop some awareness of the reality of interbeing, we get nearer to comprehend it.

Le ipotesi di Franco - Franco's hypothesis

Ritornando alla discussione su un nuovo modello medico, possiamo chiederci: ma chi insegna ai medici ad ascoltare e a comprendere i pazienti? Al momento probabilmente questo non è considerato un aspetto fondamentale della formazione medica, non solo perché non gli si dedica molto tempo ma anche perché includerlo in maniera significativa richiederebbe una profonda revisione del modo di avvicinarsi ai pazienti.
Senza voler ipotizzare nuovi e diversi curricula, grandi cambiamenti che richiedono decenni, è però possibile cominciare da subito a tenere conto dei bisogni e delle sensibilità che i pazienti del giorno d’oggi sempre più fanno presenti.
Franco da questo punto di vista ha molto da dire e da offrire: ascoltare è il primo passo, proporre un’interpretazione o una cura viene dopo e solamente in forma di ipotesi da verificare insieme, non come il “diktat” di chi “sa”.
Ascoltare vuol dire prima di tutto OSSERVARE, raccogliere indizi a cui poi si può provare a dare un senso, ma solo in un secondo momento e insieme.
Introdurre questo passo nella propria pratica di medico o di psicologo potrebbe essere una piccola ma significativa rivoluzione.
Questo primo momento, scevro da pregiudizi, libero da preconcetti, consente poi di formulare le “ipotesi” tanto care a Franco. Ipotesi, qualcosa che ci sembra di capire ma che è l’altro a dover utilizzare e solo l’altro può sapere se vanno bene per lui/lei, se hanno senso per lui/lei, o se invece serve qualcos’altro.
Tante volte abbiamo discusso di questo ed era così difficile per me all’inizio capire il suo attaccamento a questo concetto: si tratta solo di un’ipotesi che sono immediatamente disposto a cambiare, non è mai qualcosa che “so”.
Ecco, questo è così diverso dalla comune mentalità medica ormai sempre più superata, che qualunque piccolo cambiamento in questa direzione potrebbe essere di grande aiuto.

Current conundrum and making hypothesis

Coming back to our discussion about a new medical model, we can ask ourselves: is there anyone teaching medical doctors to listen and understand their patients? At the moment this doesn’t appear to be a priority in medical training, not only because of lack of time dedicated to it but also because including it significantly would require a profound revision of the common approach to patients.
Not to think about new and innovative curricula, which imply huge changes that take many years to be implemented, still it is possible to start right away to consider the needs and sensitivities that patients nowadays bring more and more forward.
Franco has a lot to say and to offer in this respect: listening is the first step, proposing an interpretation or a cure comes only afterwards and only in the form of hypothesis, something to be verified together. Not the “diktat” of someone who “knows”.
Listening implies first of all OBSERVING, collecting information to which one can then try to give a meaning, but only as a second step and together with the other.
Introducing this step in one’s own medical or psychological practice could be a small but considerable revolution.
This first moment, void of prejudice, free from pre-conceived concepts, allows the formulation of the “hypothesis” so dear to Franco. Hypothesis, something we seem to understand, which nevertheless is up to the other to use; so, only the other can say whether they are ok for him/her, whether they make sense for him/her, or whether something else needs searching.
So many times we have discussed about it; it was difficult for me at the beginning to understand why he was so fond of this concept: it’s only an hypothesis that I am fully willing to change, it’s not something that I “know”.

This is so different from the common medical way of thinking, which is becoming more and more obsolete, that any small change in this direction could be very helpful.
